Art Class Registration Forms

Art Class Registration Form - Adult

Art Camp Registration Form - Children
Use this form for registering your child for the March Break Art Camp Class or Summer Art Camp with the Lindsay Gallery.

Children's Contact and Release Form
This form is required for all children registered for our day classes. It provides the art instructors with information on any health issues they should be aware of and contact information in the event of an emergency. It is also a Release Form for you to consent to use of any pictures of your child the Gallery might take to be used in promotional materials about our children's programs.

Request to Exhibit Form

The Lindsay Gallery accepts applications to exhibit from all residents of Ontario on an ongoing basis. The opportunities for solo and joint exhibitions are limited, and are considered as space and scheduling permits. All artists applying must submit a complete package, which will be juried for acceptance by the Curatorial Exhibit Selection Committee.

Request to Exhibit Form - Showcase
The Lindsay Gallery accepts applications to exhibit artwork in our Showcase Boutique. All artists applying must submit a complete package, which will be juried for acceptance by the Showcase Selection Committee.

Juried Show Entry Forms

Deadlines - All entry forms must be received by The Lindsay Gallery by Tuesday, June 14, 2011 by 4 pm, accompanied by the appropriate entry fee.

Cheques should be made payable to The Lindsay Gallery.

All work must be delivered to the Gallery on Monday, June 27 between 10 am & 4 pm. The Gallery is located on the second floor of the Lindsay Public Library.

Artists will be notified on Tuesday late afternoon, June 28th by telephone, of works not accepted for the exhibition. These works must be picked up by the artist on Wednesday, June 29th during regular Gallery hours, 10 am to 4 pm.

Works selected for exhibition are to be picked up by the artist on Saturday, July 30 1-3pm.

 

Members Christmas Show

Annual Christmas Members Show Form - 2011 Form Now Available
Each Lindsay Gallery Member is entitled to submit one or two pieces of art for sale. Works should not exceed one square meter in size.

Board Member Application Form

Board of Governors Application Form - Applications are now being received for appointment and/or election to the Board of The Lindsay Gallery. This form can be downloaded and typed on directly and emailed back, or printed on and mailed back.
